What it does

This project is a joint venture between Social Prescribers, Bradford City Council, Community Wardens and St John’s Church.

Social Prescribing enables GP’s, nurses and other primary care professionals to refer patients to a link worker who will assist them to seek access to advice for welfare rights, housing and any other health care needs.
Due to a blend of skills such as spoken languages, faith and non-faith communities attend the provision at the Church to acquire basic food items,  personal hygiene products, seek access to advice for welfare rights, housing and any health care.


The Social Prescribing Link Workers role is to facilitate the process, this extends to befriending users of the service and a working relationship where possible.
